Case Study: How Fibo Cloud Built Bhutan’s First National Digital Health Tracking Infrastructure

When discussing tech export from emerging markets, conversations often revolve around software outsourcing or developer staffing. But at Fibo Cloud, our vision has always been different: to engineer mission-critical, national-scale digital infrastructure that solves real societal problems.

Between late 2023 and early 2026, in partnership with the World Bank Group, the Japan Social Development Fund (JSDF), and the Royal Government of Bhutan, Fibo Cloud architected, deployed, and handed over the core digital backbone for Bhutan’s flagship maternal and child health initiative: the Accelerating Maternal and Child Health Program (AMCHP) – 1000 Days Plus.

Here is the story of how our team took Mongolian engineering to the Himalayas.

The Mission: Safeguarding the “Golden 1,000 Days”

The first 1,000 days of life—from conception through a child’s second birthday—form the foundation of lifelong health, cognitive development, and well-being. To incentivize vital healthcare milestones such as regular antenatal checkups, institutional deliveries, and post-natal immunizations, the Royal Government of Bhutan introduced a national Conditional Cash Transfer (CCT) program.

The missing link was the infrastructure:

  • No legacy digital foundation: Bhutan had never operated a centralized, paperless digital platform for maternal-child cash transfers.
  • Geographic fragmentation: Primary health centers are scattered across rugged Himalayan valleys, isolated villages, and high altitudes.
  • Complex verification: Cash payouts had to tie directly and reliably to certified clinical interventions while maintaining strict auditability.

Fibo Cloud was commissioned to engineer the Tracking and Information System (TIS)—the nationwide platform responsible for tracking clinical milestones and automating beneficiary verification for timely fund disbursement.

The Build: Engineering for Resilience and Frontier Environments

Kicking off development in December 2023, our engineering team focused on creating an intuitive, high-availability platform tailored to the realities of frontier healthcare delivery:

  1. Frontline Simplicity: Built for community health workers on varying hardware specs and variable network conditions, ensuring low latency and clean user flows during patient consultations.
  2. Automated Verification: Seamlessly mapped clinical eligibility against conditional payout rules, minimizing human error and processing delays.
  3. Data Security & Sovereign Compliance: Designed within strict data-governance standards required by both the World Bank and the Royal Government of Bhutan.

Nationwide Rollout & Scale

Deploying software at a national scale requires operational endurance. Over a two-year rollout across the country, the project delivered measurable outcomes:

  • 100% District Coverage: Reached all 20 districts (dzongkhags) across Bhutan.
  • 94% Primary Health Facility Adoption: Successfully onboarded 274 primary healthcare centers, linking remote clinics directly to the central ministry.
  • 274 Healthcare Workers Trained: Conducted exhaustive system training across the country to ensure autonomous daily operations by local staff.
  • Continuous Engineering Support: Our engineering team provided remote monitoring, real-time bug fixing, and continuous user-experience refinements through the end of the technical support period on March 31, 2026.

Building for Independence, Not Vendor Lock-In

True public-interest technology must be self-sustaining. From day one, the goal was not to make Bhutan dependent on an external vendor, but to leave behind a robust, fully autonomous national asset.

In early 2026, we completed the final handover to the GovTech Agency of Bhutan. The comprehensive handover package included:

  • Complete production source code and architectural documentation
  • Relational database schemas and API specifications
  • Deployment playbooks, CI/CD pipelines, and infrastructure setup guides
  • Detailed administrator and user training manuals

With this transfer, Bhutan’s internal GovTech and Ministry of Health teams possess full ownership to maintain, patch, and extend the system for decades to come.

The Broader Picture: Mongolia on the Global Tech Map

For Fibo Cloud, the AMCHP TIS project was more than a successful contract—it validated that software engineering teams from Mongolia can compete internationally, meet World Bank institutional standards, and deliver core public infrastructure that impacts hundreds of thousands of lives.

It serves as a foundational chapter in our export playbook: building resilient, secure, and human-centric software with an open, reinventive mind.
